Black Ice’s Form Designer is a Desktop application to design, edit or create medical, governmental, financial, and fax forms to be filled and reused. The forms are fillable with the Form Filler. Once the forms are filled out, the form can be saved in TIFF or PDF format. The form designer uses drag and drop fields in the design process. Using annotations is an option too, to make the documents more expressive.
There are thousands of forms and templates available in PDF, Microsoft Word, and Microsoft Excel format that can be quickly converted into fillable Forms with the Form Designer.
